Appointments

All office visits are scheduled by appointment only.

We appreciate that your time is valuable. To make your visit as efficient as possible, we ask that you arrive a few minutes in advance of your appointment. We make every effort to keep precisely to our daily schedule but we will adjust the schedule when a patient arrives in pain or when a treatment takes more time than expected. We appreciate your understanding and cooperation.

If circumstances require dramatic changes to the schedule, we will do our best to notify you as far in advance as possible. If you are unable to make your appointment, please let us know right away and we will be happy to reschedule.

Your First Office Visit

In order to prevent dental disease, the active disease must be evaluated and eliminated. Therefore, we must have complete picture of your present oral condition, as well as insight into your dental and medical history. We will make a thorough evaluation of existing and potential problems, taking the necessary x-rays and examining the oral tissues for decay and gum/bone disease. A complete head and neck oral cancer screening exam will also be performed.

Please assist us by providing the following information at the time of your visit:

  • A description of your problem, if any.
  • A list of medications you are currently taking.
  • Any need for antibiotic pre-medication.
  • Any current x-rays.
  • Completed Patient Information and Patient History forms.
  • Dental Insurance Information.

Our forms are online! If you would like to complete your patient forms before arriving at the office, please download the PDF forms below, print them, complete them, and bring them with you on your first visit. Please note that both forms are required.

Note: The PDF forms are about 500KB each in size. Please allow ample time for the download. You will need Adobe Reader to view the files. You can also use Adobe Reader to fill in the form and then print it, but be advised that you can not save the form with the filled-in information - when you exit Adobe Reader, the contents will be lost.

If you have any questions about the forms, please give us a call and we will be happy to assist you. If you wish to complete your forms during your first visit, please allow a few extra minutes in advance of your appointment.

IMPORTANT: Please notify our staff if you have a medical condition (diabetes, high blood pressure, rheumatic fever, artificial heart valves and joints, etc.) that may be of concern prior to your dental visit, especially before dental surgery or if you are on any medications like aspirin, heart medications, anticoagulant therapy, etc..

Safety

Our office practices the highest level of safety possible to assure everyone's protection. Treatment rooms are cleaned and disinfected after each patient. All instruments are cleaned, disinfected and sterilized as recommended by the American Dental Association (ADA) and the Centers for Disease Control (CDC). Whenever possible, we use disposable, single-use items.
